New HERDS Survey Questions

NYSHFA Participated in a NYSDOH Webinar Regarding New HERDS Questions

There will be a new section regarding COVID-19 Vaccination on the HERDS survey beginning Monday, December 21, 2020.

The first group of questions refer to the yesterday time period (12:00 am – 11:59 pm).  For that group of questions, providers need to first report the number of residents that received the vaccination the day before.  The question is then broken down by asking of the residents that received vaccination, how many received their first dose, and how many received their second dose?  Finally, of those residents who received their first dose, did they receive it onsite or offsite?  Of those residents who received their second dose, did they receive it onsite or offsite?

The same questions and breakdown are then asked of staff.

The second section requests cumulative data.  For this group of questions, the provider will report the total number of residents that have received their first dose of vaccination.  They will then report the total number of residents that have received both the first and second dose of vaccination.  Finally, they will report the total number of residents that declined vaccination.

The same questions are then asked of staff.

Please note there are several rules built in to the survey to help guide in answering the questions accurately.  If your totals do not match, you will receive an error code explaining what needs to be corrected.  You will not be able to move forward in the survey until the item(s) are corrected.

If no residents or staff received the vaccine during the yesterday time period, the facility would report “0.”

As always, the survey is due by 1:00 PM each day.  If residents or staff are vaccinated after submission of the survey, they would be reported the following day.
