ECHO MOLST: Honoring Patient Preferences at End-of-Life

ECHO MOLST will be hosting an 8-week videoconferencing clinic series beginning September 18, 2019. 1-hour cased-based clinics will be held for 8 consecutive weeks on Wednesdays at noon with free CME credits for attendees. If you haven’t already, please take a look at the ECHO MOLST flyer that gives a high level overview of the goals of the ECHO MOLST series and details about clinic dates and times. ECHO MOLST will run on Wednesdays continuously for 8 weeks beginning September 18, 2019 and ending November 6, 2019. Clinics will take place from 12:00-1:00pm EST.

In order to register for this clinic, complete the following steps:

1) Read the Welcome Guide

2) If your organization hasn't already done so, complete and sign the Participation and Collaboration Agreement and return it to me via email. Only 1 agreement per organization is needed.

3) Enroll all individuals from your organization who will be participating in this clinic series. Participants from previous ECHO MOLST Clinics will need to re-enroll. Here is the enrollment link: https://formstack.io/F457D. Once registered, you'll receive a confirmation email that will prompt you to complete the ECHO MOLST pre-test and will include a link to download all of the ECHO MOLST clinic dates with Zoom login information to your calendar.

4) If this is your first time participating in ECHO MOLST, consider attending an onboarding Session. In this session, we'll test the Zoom videoconferencing platform and your audio connection. You'll also learn more about the ECHO model and understand what a typical ECHO MOLST clinic session will look like. Session date/time and connection info are listed below. Please RSVP for a session by contacting me at Meg.Greco@Excellus.com and I will send the Zoom login information.

5) For organizations that will have a large team (5 or more individuals) joining from 1 site, assign an IT contact from your organization and have them complete the Spoke IT Equipment and Support Form. Email the completed form to Katie Orem at Katie.Orem@Excellus.com.
