Join NYSHFA/NYSCAL on June 28th at 10am for Post-Acute Care Medication Management (A Requirement of Participation) Audio Conference. As caring for skilled nursing residents with shortened lengths of stay and greater acuity increases in complexity and federal regulations intensify, providers will require pharmacy skills and services that improve medication safety and achieve quality, operational and financial goals. This session explores the value of pharmacist-provided medication reconciliation during transitions of care. The speaker will share the impact pharmacists have on resolving MRPs, discontinuing unnecessary or potentially inappropriate medications, improving medication safety, streamlining individualized care, reducing re-hospitalization rates and decreasing total cost of care. Attendees will be exposed to effective strategies for utilizing pharmacists in medication management services that increase workflow efficiencies, reduce nursing workload, contribute to financial sustainability and allow PAC executives to optimize medication safety, meet regulatory standards and achieve quality goals.
