Age Friendly Health Systems

Lisa Volk and Nancy Leveille

The Institute for Healthcare Improvement is launching their second Age-Friendly Health Systems Action Community. They have let us know that there is still room for additional participants.

This Acton Community begins on April 1. The Action Community is a free, seven-month learning community run by the Institute for Healthcare Improvement.  It succeeds the first Action Community of teams from more than 75 systems across the nation working together to rapidly test and scale the Age-Friendly Health Systems 4Ms Framework. For more information, download this invitation.

Age-Friendly Health Systems is an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement in partnership with the American Hospital Association and the Catholic Health Association of the United States. For more information, visit
