HERDS Critical Asset Survey - Reactivated for Q1 2019 Review/Update

This message is to inform all Health Care Facilities (HCFs) - hospitals, nursing homes (NHs) and adult care facilities (ACFs) - that the Critical Asset Survey in HERDS has been reactivated for the Q1 2019 review/update.

The three forms of the Critical Asset Survey (Baseline/Surge/Resilience) have been merged into one, combined form going forward. All data that was previously submitted in both the Surge and Resilience forms has been migrated to the new, combined form and therefore does not need to be re-entered. 

This Critical Asset Survey reactivation comes in conjunction with the rollout of the Facility Evacuation Planning Application (FEPA) in New York City. A Dear Administrator Letter (DAL) regarding FEPA (previously known as the Facility Profile Application) was sent to HCFs in NYC last Friday (12/28/18). 

The Critical Asset Survey review and update period runs from January 1 - March 31, 2019. All Health Care Facilities are required to review their previously submitted data, update any that has changed and (re)Submit the Critical Asset Survey data during this time period.
