DOH Requiring NYC Adult Care Facilities to Update Facility Profile Application

The Department is requiring all NYC-based healthcare facilities to review and attest to the accuracy of, or make needed updates to, and submit the required data in the FPA no later than March 31, 2018.  NYS DOH has been joined by NYCDOHMH staff and will be contacting NYC facilities to provide technical assistance in the use of the Facility Profile Application.  The letter from the Department and training schedule is listed below.  To access the Facility Profile Application v 2.0 Users Guide, click on the link below and you will be prompted to log into your Health Commerce System Account (HCS).
