The New York State Department of Health (NYSDOH) – Facility Profile Application (PA) is a planning tool that provides health care facilities (HCF) (adult care facilities, hospitals, nursing homes) with an easy process to assess and maintain information on key preparedness capabilities and capacities. This includes detailed information about the facility’s patient/resident send – receive arrangements with other HCFs as part of their evacuation planning. The application was made available to ALL New York City (NYC) HCFs in July 2015. The PA has undergone extensive improvements and upgrades in functionality since its initial release, based largely on lessons learned in support of HCFs using the PA. This webinar will initiate the release and training for PA Version 2.0! See attached for details.
