Informational Message: Healthcare Personnel Influenza Vaccination Report - Due Date May 1, 2017

Author: Lisa Volk and Nancy Leveille* Published in Clinical & Quality

This is a one-month reminder that the annual required Healthcare Personnel Influenza Vaccination Report is due to the New York State Department of Health (NYSDOH) by close of business May 1, 2017. If you are not responsible for submitting this report for your healthcare facility or agency, then please send this message to the individual responsible for submitting the report.

As soon as possible, but no later than April 30, 2017, please log in to the Health Commerce System and enter your facility or agency's data in the Healthcare Personnel Influenza Vaccination Report. If your facility has already saved or submitted data in the report, please take this opportunity to review the data for accuracy and update or correct data as necessary. The report can be updated and re-submitted as many times as necessary until May 1, 2017. Users are reminded to first Save, then press the Review and Submit button in order to successfully submit data to the NYSDOH. Pressing the Save button alone does NOT submit data to NYSDOH.

This report will close on May 1, 2017 and it must be submitted through the Health Electronic Response Data System (HERDS) on the Health Commerce System. Questions about the Healthcare Personnel Influenza Vaccination Report should be directed to the NYSDOH Bureau of Immunization at either (518) 473-4437 or Technical questions about the report or on the use of HERDS should be directed to the Health Emergency Preparedness Program at (518) 408-5163 or
