Hospitals, ACOs and managed care plans are evaluating post-acute strategic partnerships. This session will explore what you, as a
post-acute provider, need to do to establish and maintain these relationships. The session will explore 10 criteria acute care partners are expecting and what you need to consider in terms of your nursing staff model while preparing your staff for this changing environment. Learn what a successful nurse leader has done to prepare for and maintain relationships and gain the confidence of hospitals and managed care providers. This session will highlight tools for acuity based staffing, strategies for reducing re-hospitalizations, ensuring patient
satisfaction as well as looking at a model for root cause analysis.

This session offers updates on emerging industry trends, state regulatory requirements, changing payment models, Medicaid funding for long term care services and supports, and the implementation of the final rule defining home and community based settings under the Medicaid waiver program. Several initiatives being undertaken by federal departments and agencies that affect assisted living will be presented. Risk management and key quality initiatives that help assisted living providers thrive in a changing marketplace will also
be discussed.

Attendees will learn the following: 1) An understanding of what activities federal policymakers are undertaking that impact assisted living; 2) State and national industry, regulatory, and legislative trends and developments; and 3) Current quality initiatives and data collection efforts to help assisted living professionals succeed now and in the future.
