DOH: Facility Profile Application Updates for 2016 Coastal Storm Planning

DOH sent out the following notice this week related to updating your Facility Profile application. With the different storms events we have had over the years, we need to keep this information current and also DOH notes it should reduce many of the surveys they end up sending out to you during the crisis. Please see their notice below:

"Facility Profile Application Updates for 2016 Coastal Storm Planning"

The Profile Application (PA) was designed as a planning tool to facilitate the development and maintenance of health care facility (HCF) evacuation planning information. The tool includes information on evacuating and receiving facilities and the send-receive arrangements between them. The application eliminates the need for health care facilities to repeat lengthy annual coastal storm planning surveys. It is designed to be used in conjunction with and does not replace direct facility to facility dialogue to develop send-receive arrangements.

To access the Profile Application go to the Health Commerce System (HCS) site.

The PA consists of several different easily navigated windows through which health care facilities (adult care facilities, hospitals and nursing homes) can monitor and update their planning information.

As we kick off the 2016 Coastal Storm Planning Season all health care facilities will need to do the following:

Assign a Facility Profile Coordinator, if you haven't already assigned this role

Access, review and update your facility's Profile Data

Access, review and update your facility's Send/Receive arrangements

New York State Department of Health, Office of Health Emergency Preparedness (OHEP) will follow up with health care facilities (adult care facilities, hospitals and nursing homes) in approximately one month who haven't assigned a role and/or reviewed their facility information.
