Register Now for Quality Improvement Series to Begin in early 2016!

The Foundation for Quality Care, the research and education affiliate of the New York State Health Facilities Association (NYSHFA), is pleased to announce that the Long Term Care Leadership Institute will be offering a Quality Improvement Series Leadership program in East Elmhurst, NY.  Applicants may apply who work in Skilled Nursing Facilities from the following New York City, Long Island, and Northern Metro counties: Bronx, Columbia, Delaware, Dutchess, Kings, Nassau, New York, Orange, Putnam, Queens, Richmond, Rockland, Suffolk, Sullivan, Ulster, or Westchester.  A brochure, application and program information sheet outlining the course content, eligibility criteria, dates, time and location for these programs is listed on and also can be downloaded HERE

Applications and supporting documentation must be received by the Foundation for Quality Care no later than Friday, January 8, 2016.

2015 Nursing Home Quality Initiative Results: Check Your Results!

2016 NYSHFA Employee Recognition Program

The Clinical and Quality Services Committee is pleased to announce a call for nominations for the annual Employee Recognition Program. The goal of this program is to honor exemplary employees working in skilled nursing facilities. The four categories for awards are Registered Nurse, Licensed Practical Nurse, Certified Nursing Assistant and the “Heart and Hands” Award for non-nursing/non-managerial staff.

Attached for your facility’s convenience is the Employee Recognition Program Brochure which contains the nomination form. We have also included the Guideline to Preparing Applications.

The Clinical and Quality Services Committee will be accepting nominations until January 15, 2016.  Recipients of the awards will be honored at the Awards Ceremony during the NYSHFA/NYSCAL Annual Convention, June 26 – 29, 2016 at The Sagamore Resort in Bolton Landing, NY.

Deadline Approaching for 2016 Innovative Practice Award Program

The Clinical and Quality Services Committee, in recognition of the importance of innovation in improving the quality of long term care services, invites our members to submit nominations for the 2016 NYSHFA/NYSCAL Innovative Practice Awards.  Up to five awards will be presented.  Suggested areas for consideration are:  1) Behavioral Care Planning and Activities Programming, 2) Wound Care, 3) Preventing Unnecessary Hospitalizations, 4) Infection Control, 5) Changing to a Culture of Person-Centered Care, and 6) Staffing/Recruitment/Retention. Facility representatives from each award category will be honored at the NYSHFA/NYSCAL Annual Convention in June 2016.  Honorees will present their innovative practices to the membership during an educational session at the convention.

Attached are the 2016 Innovative Practice Awards Brochure and Nomination Form and the Guidelines to Submitting Applications. Realizing the time constraints everyone has, we have attempted to make the submission process as easy as possible. We have also included a suggested timeline you may find useful in preparing your application.  Part one of the submission form contains your facility's identifying information. It will be separated from Part Two, the description of the innovative practice, before the committee reviews it. Nominations will be reviewed using a "blind" process; therefore, do not include any identifying information when completing Part Two of the submission.

Submissions must be received by mail no later than December 18, 2015.
