AHCA is asking for your input related to the activities you have initiated for Person Centered Care in your facility/organization. Please review the following information from AHCA and complete the attached survey. There are 40 questions, many of which can be easily answered. We hope you will share the positive work that is being accomplished in SNF across NYS.

“It’s been almost a decade since the Commonwealth Fund sponsored a national survey to assess the adoption of Person-Centered Care [PCC] Practices in nursing homes across the US. It is, once again, time to take the pulse of the industry as well as explore newer concepts and practices that have gained traction. The Rothschild Foundation, in collaboration with Perkins Eastman, IDEAS Institute, and The Beryl Institute, has developed the 2015 State of the Art of Person-Centered Care Survey.

We need you to share the changes you have been making in this field. The 2015 State of the Art of Person-Centered Care Survey will be available online from November 1 through November 23. It can be accessed at www.IDEASInstitute.org/2015PCCSurvey. While you may receive this link from more than one partner, there is no need to complete the survey more than once.
