Informational Message: Final HCP Flu Vaccination Report Reminder - Due 5-1-15

Karen Morris in Clinical & Quality

This is your final reminder from the New York State Department of Health (NYS DOH) that the required Healthcare Personnel Influenza Vaccination Report must be electronically submitted on the Health Electronic Response Data System (HERDS) by close of business May 1, 2015. This report is required pursuant to the flu mask regulation. No extensions will be granted.

Please ensure that the individual responsible for reporting for your facility or agency accesses the report on HERDS, ensures that the data is complete and accurate, and submits the report prior to close of business May 1, 2015.

To access the report, click on Data Entry in HERDS, choose the Activity named "Health Care Personnel Influenza Vaccination Report", and select the reporting facility or agency from the Organization dropdown menu. If a closed or incorrect facility or agency is listed in your HERDS report Organization dropdown menu, please contact the NYS DOH for correction.

Users are reminded to first Save, then press the Review and Submit button in order to successfully submit data to NYSDOH. Pressing the Save button alone does NOT submit data to NYSDOH.

Detailed guidance on how to access, complete, and submit the report on HERDS, including instructions and a Frequently Asked Questions (FAQ) document, are available on the NYS DOH website at http://www.health.ny.gov/prevention/immunization/health_care_personnel/docs/dal_hcp_report_faq.pdf. Questions about the report should be directed to the NYS DOH Bureau of Immunization at either (518) 473-4437 or immunize@health.ny.gov. Technical questions about HERDS should be directed to the Health Emergency Preparedness Program at (518) 408-5163 or hseppny@health.ny.gov. Questions about the "flu mask regulation" should be directed to flumaskreg@health.ny.gov.

*Note: For Hospitals and Diagnostic and Treatment Centers (DATC) with extension clinics all extension clinic staff should be included in the main hospital or DATC report. See questions #7 and #8 in the FAQs.
