Healthcare Personnel Flu Vaccine Report Due May 1st

The New York State Department of Health (NYS DOH) issued a reminder that the required Healthcare Personnel Influenza Vaccination Report must be electronically submitted on the Health Electronic Response Data System (HERDS) by close of business May 1, 2015. Please ensure that the individual responsible for reporting for your facility or agency accesses the report on HERDS, ensures that the data is complete and accurate, and submits the report prior to close of business May 1, 2015.

To access the report, click on "Data Entry" in HERDS, choose the Activity named "Health Care Personnel Influenza Vaccination Report", and select the reporting facility or agency from the "Organization" dropdown menu. If a closed or incorrect facility or agency is listed in your HERDS report "Organization" dropdown menu, please contact the NYS DOH for correction.

Users are reminded to first Save, then press the Review and Submit button in order to successfully submit data to NYSDOH. Pressing the Save button alone does NOT submit data to NYSDOH.

Questions about the flu mask regulation should be directed to flumaskreg@health.ny.gov.
