Mandatory Final Annual Immunization Report due May 2nd – **Note Changes

This year’s annual (final) Healthcare Personnel Influenza Vaccination Report will be opened on the HCS on April 1st and must be completed electronically by Friday May 2, 2014.  See attached DAL and FAQ document.

This year’s final report, which covers the period from September 1, 2013 to March 31, 2014 is a cumulative report reflecting the entirety of the reporting period, including personnel reported in the previous (November 2013) report. 

Please note, the Influenza/Pneumococcal Vaccination and Outbreak Report that was previously required on May 1st, is no longer being used.   NYSDOH is now able to receive resident influenza and pneumococcal vaccination rates directly from the MDS assessments.  In order to ease the administrative burden on facilities, the NYSDOH has eliminated the resident vaccination component from the long term care facility immunization report to NYSDOH.  The Healthcare Personnel Influenza Vaccination Report is now the only required immunization report to the NYSDOH for long term care facilities.
DOH Quality Pool - The HCP Influenza Vaccination Report will impact the Nursing Home Quality Pool in two metrics. The first is the compliance measure of timely submission of the two reports due for the 2013-2014 Influenza season (November and March.) The 5 points for timely submission will be split into 2.5 points for each report. The second measure is the quality measure for the rate of employee vaccination which will be calculated using the second (final) report.

New York State nursing homes are required to submit their annual reports electronically. The survey must be completed on the Health Electronic Response Data System (HERDS3) on the Health Commerce System (HCS.)
