For New York City Members Only - Nursing Home Discharge Planning Survey

The Center for Independence of the Disabled, NY (CIDNY), with funding from the New York Community Trust, is conducting a survey project in New York City to evaluate the process related to the Section Q of the Minimum Data Set (MDS). Question Q0500B requires a nursing home staff person to ask all residents about their desire to talk to someone about returning to the community. Once a resident replies, "yes" to the question, a referral is sent to the local contact agency, which is mandated to meet with the resident and discuss options for care in the community.

The survey asks nursing home staff charged with asking residents this question, to share their experiences, concerns, and ideas for improving the system. CIDNY will issue a report in December with recommendations for improving the process, and is seeking the input of nursing home staff. CIDNY assures all answers are confidential.

The online survey is open until April 30.
