Title:  Nursing Home Quality Improvement Projects: Dementia Training and Edge Projects

Description:   The intent of this RFA is to support CMS initiative and nursing homes that aim to reduce off-label use of antipsychotic medications by providing the facilities with funds to implement one or more Dementia Grants Program projects and/or EDGE interventions.  Applicants are urged to approach this project as a quality improvement project.

Eligible Applicants:  Only nursing homes licensed under Article 28 of New York’s public health law are eligible to apply for funds.  All proprietary, public, and not-for-profit nursing homes are eligible.  NOTE:  Preference will be given to nursing homes whose use of antipsychotic drugs for long stay residents is above 21% (the statewide average for 2012 quarters 1-3 as shown on the Department’s web site (http://nursinghomes/nyhealth.gov/).

Funding Available: $611,000 is available for 22 grant awards for two years starting March 1, 2014.   Maximum Award Size: $28,000
