Mandatory Annual Immunization Report Submission via HCS due May 1st

Skilled nursing facilities are among those required by NYS PHL 2190-2196 to document their immunization (influenza and pneumococcal) efforts and to submit reports electronically to the New York State Department of Health (NYSDOH)on an annual basis. (See attached DAL.) This year’s annual report, which covers the period April 1, 2012 to March 31, 2013, is due to the NYSDOH by May 1, 2013. Please note that timely completion of the Influenza/Pneumococcal Immunization Data Report (i.e., by May 1, 2013) is a compliance measure in the 2013 Nursing Home Quality Pool. Points will be awarded for timely submission of immunization data. Nursing homes that fail to complete the Immunization Data Report by the May 1 deadline will be ineligible for Nursing Home Quality Pool points for timely report completion. Refer to the attached DAL for further information related to the Quality Pool and Quality Measures related to the Immunization Reporting requirement.

New York State nursing homes are required to submit their annual reports electronically. The survey must be completed on the Health Electronic Response Data System (HERDS3) on the Health Commerce System (HCS.) The report may be accessed on the HCS at: https://commerce.health.state.ny.us/. DOH will conduct webinars to review the HCS and data submission process. Participants may register for the live webinar at https://nysdoh.webex.com/. The webinar will be recorded and posted online shortly after the live webinar at http://www.health.ny.gov/prevention/immunization/providers/webinar_series.htm. Please ensure that the person completing the report at your facility either attends the live webinar or views it online.
